NOTE:
A.	If your tournament is going to be more than one classification, you must submit a separate application for each classification.
B.	The team classification indicates which teams may participate in the tournament. Recreational Outdoor teams may only participate in R classifications. Competitive Amateur Outdoor teams may only participate in C or O classifications. Professional outdoor teams may only participate in P or O classifications. Amateur Indoor Teams may only participate in I and O. Professional Indoor Teams may only participate in PI and OI.
C.	The team type indicates whether the tournament is for Club Teams, All-Star Teams, or Indoor Teams. Club Teams may only participate in C Type tournaments, Competitive All-Star Teams may only participate in CA Type tournaments. Recreational Teams (All-Star and Select) may participate in C Type Recreational Tournaments, provided the tournament rules permit that Team Type. However, a Recreational Club League All-Star Team is only eligible to play in an Inter-District or District tournament for Recreational Club Leagues teams.
D.	The Tournament Application Fee amount can be obtained from your District Association.
